CHAPTER ONE

GENERAL INTRODUCTION

 1.1 Introduction

Immunization campaigns are critical public health initiatives aimed at preventing infectious diseases and protecting community health. In the North West region, these campaigns face unique challenges and opportunities shaped by local demographics, cultural beliefs, and healthcare infrastructure. Understanding community responses to immunization efforts is essential for enhancing vaccine uptake and achieving herd immunity.

Communities often exhibit varying degrees of acceptance or resistance to vaccination, influenced by factors such as historical experiences with healthcare systems, misinformation, and trust in governmental and health authorities. Engagement with community leaders and stakeholders is crucial in addressing concerns, dispelling myths, and fostering a supportive environment for immunization.

This introduction sets the stage for a deeper exploration of how different communities in the North West region respond to immunization campaigns, examining case studies, public perceptions, and the effectiveness of communication strategies used to promote vaccination. By analyzing these responses, we can identify best practices and develop tailored approaches to improve immunization coverage and health outcomes in the region.

Immunization campaigns are crucial in preventing and controlling vaccine-preventable diseases. In Bamenda, North West Region (NWR) of Cameroon, these campaigns have been implemented to improve vaccination coverage and reduce morbidity and mortality. However, community responses to these campaigns have been varied, with factors such as cultural beliefs, trust in healthcare providers, and access to healthcare services influencing their effectiveness.

A community can be defined as a group of individuals who share common characteristics, interests, or goals and who often interact with one another within a defined geographical area (MacQueen et al., 2001). Communities can take various forms, including interest based, geographical and cultural. Overall, communities play a vital role in shaping individual identities, providing support networks, and fostering collaboration for common goals. Immunization is a critical public health intervention aimed at preventing infectious diseases, particularly among vulnerable populations such as children and pregnant women. In Cameroon, the Expanded Program on Immunization (EPI) was introduced in 1976 to combat vaccine-preventable diseases. Despite these efforts, immunization coverage remains suboptimal, particularly in regions like the North West, where Bamenda is located. The North West region faces unique challenges, including geographical barriers, inadequate health infrastructure, and socio-cultural factors that hinder effective immunization campaigns (Njim et al., 2015). Bamenda, as the capital of the North West region, serves as a focal point for health initiatives. However, the region has experienced fluctuations in immunization rates, often attributed to logistical issues such as the cold chain management for vaccines, which is essential for maintaining vaccine efficacy (BMC Research Notes, 2013). The cold chain system in many health facilities is often compromised due to inconsistent power supply and lack of functional equipment, leading to concerns about the safety and effectiveness of vaccines administered during campaigns (BMC Research Notes, 2013). Immunization is a critical component of public health, and community involvement is essential for the success of immunization campaigns. Bamenda, the capital of the North West Region (NWR) of Cameroon, faces unique challenges in achieving high immunization coverage. This background study aims to provide an overview of community responses and immunization campaigns in Bamenda, NWR. Despite efforts to improve immunization coverage, Bamenda still experiences challenges in achieving high immunization rates. According to the 2018 Cameroon Demographic and Health Survey (CDHS), the immunization coverage for children aged 12-23 months in the North West Region was: 73.4% for BCG (Bacillus Calmette-Guérin), 64.1% for DPT3 (Diphtheria, Pertussis, and Tetanus).

Community responses refers to the reactions,actions or behaviors of a group of people living in a specific geographical area or sharing common interests,values,or characteristics in response to a particular situation ,event or issue.some examples of these responses include Health crises,social issues and Environmental concerns ,characterized by collective action,shared values and interest.

  Immunization is crucial for preventing infectious diseases, reducing morbidity and mortality, promoting public health, protecting vulnerable populations, and ensuring global health security. By protecting against vaccine-preventable diseases, immunization decreases illness and death, improves overall health and well-being, and safeguards children, older adults, and individuals with compromised immune systems. Ultimately, immunization prevents outbreaks, reduces healthcare costs, and saves lives, promoting health equity and preventing pandemics.

 1.2 Statement of the problem.

According to Nkwain, P.A et al,. (2021)Despite the proven efficacy of immunization in preventing infectious diseases, community responses to immunization campaigns in the North West region remain inconsistent and often challenging. Many communities exhibit varying degrees of acceptance, resistance, or indifference to vaccination efforts, leading to suboptimal immunization coverage. Factors contributing to this phenomenon include cultural beliefs, historical distrust of healthcare systems, misinformation, and socio-economic barriers. This variability in community response not only jeopardizes individual health but also threatens public health efforts aimed at achieving herd immunity. The lack of understanding and engagement within communities can result in significant health disparities, increased disease prevalence, and higher healthcare costs Ndongmo W.M et al, (2021). Therefore, it is crucial to investigate the underlying reasons for differing attitudes toward vaccination and to identify effective strategies for improving community engagement and vaccine uptake.

1.3 Research questions.

How do cultural beliefs and trust in healthcare providers affect community participation in immunization campaigns in Bamenda NWR ?

  1. What are the primary cultural beliefs, community perceptions that influence community hesitancy towards immunization campaigns?
  2. How do community leaders and influencers shape perceptions and acceptance of vaccines within their communities?

1.4  Research objective

Main research Objective

To identify and analyze the factors that influence community attitudes and participation in immunization campaigns, with a focus on cultural beliefs and trust in healthcare providers.
