GENERAL INTRODUCTION

1.0 Introduction

Maternal health is a cornerstone of global public health, profoundly influencing maternal and neonatal outcomes. According to the World Health Organization, complications during pregnancy and childbirth claim approximately 295,000 lives annually, with the vast majority of these preventable deaths occurring in low- and middle-income countries. Effective maternal care, particularly during pregnancy, plays a critical role in identifying and mitigating risk factors such as preeclampsia, gestational diabetes, and infections that significantly impact maternal and neonatal health. These interventions are particularly essential in sub-Saharan Africa, where healthcare disparities and resource limitations exacerbate maternal morbidity and mortality rates. Nurses occupy a pivotal position in antenatal care, functioning as primary caregivers and educators in risk factor management. Their responsibilities encompass early detection of complications, timely referrals, and health promotion through education on nutrition, hygiene, and lifestyle modifications. By implementing evidence-based practices, nurses contribute to reducing pregnancy-related complications, as highlighted by studies emphasizing the importance of skilled care before, during, and after childbirth.

Nkwen District Hospital, a prominent healthcare facility in Cameroon, exemplifies the challenges and opportunities in antenatal care delivery in resource-constrained environments. The hospital provides essential maternal health services to a population facing systemic barriers, including inadequate infrastructure, limited healthcare funding, and insufficient staff training. This makes it a valuable case study for examining the efficacy of nursing practices in managing pregnancy risk factors.

1.1 Background of the Study

Maternal healthcare has evolved significantly over centuries, shaped by cultural, scientific, and political forces. Historically, childbirth and maternal care were predominantly managed by traditional birth attendants, whose practices were deeply rooted in cultural beliefs. In colonial settings, such as South Asia and Africa, maternal healthcare practices became medicalized, often sidelining indigenous knowledge systems while introducing institutionalized healthcare systems [1]. In the early 20th century, international attention to maternal mortality and morbidity grew, spurring public health reforms and the establishment of maternal health frameworks [2]. Despite these advancements, the global maternal mortality rate remains unacceptably high, with developing countries disproportionately affected.

Globally, maternal health has been recognized as a critical public health priority. Sustainable Development Goal (SDG) 3 aims to reduce maternal mortality to less than 70 per 100,000 live births by 2030. While progress has been made, sub-Saharan Africa accounts for nearly two-thirds of all maternal deaths worldwide due to limited access to skilled healthcare, inadequate infrastructure, and sociocultural barriers [3] Research highlights that the primary causes of maternal mortality hemorrhage, sepsis, hypertensive disorders, and obstructed labor are preventable through timely and skilled interventions [4]

In resource-constrained settings like Cameroon, the challenges of maternal healthcare are particularly pronounced. The nation grapples with a maternal mortality rate of 529 deaths per 100,000 live births, far exceeding the global average. Rural areas face additional barriers, including limited healthcare facilities, a shortage of skilled healthcare providers, and sociocultural factors that delay care-seeking behaviors [5] These issues underscore the importance

of effective antenatal care and skilled healthcare delivery in reducing maternal and neonatal complications.

Nurses, as frontline healthcare providers, play a pivotal role in managing pregnancy-related risk factors. Their responsibilities extend beyond clinical care to encompass health education, counseling, and advocacy for maternal health. Studies indicate that skilled nursing interventions, such as early detection of preeclampsia, gestational diabetes management, and infection control, significantly reduce pregnancy complications [6] However, nurses in developing countries face numerous challenges, including inadequate training, high patient-to- provider ratios, and a lack of essential resources [7]. These constraints hinder their ability to deliver optimal care, necessitating innovative strategies and policy reforms.

Nkwen District Hospital serves as an ideal case study for exploring these issues. Located in a semi-urban area of Cameroon, the hospital provides maternal health services to a diverse population, including women from rural and underserved communities. Its antenatal care program addresses a broad spectrum of pregnancy-related risk factors, making it a critical setting for studying nursing practices in maternal health. However, like many healthcare facilities in Cameroon, Nkwen District Hospital faces challenges such as staff shortages, inadequate medical supplies, and high patient loads. These limitations highlight the need to examine the effectiveness of current nursing practices and identify areas for improvement.

This study seeks to explore the effective practices of nurses at Nkwen District Hospital in managing pregnancy-related risk factors. By contextualizing these practices within the broader challenges of maternal healthcare in Cameroon, the research aims to provide actionable insights for improving maternal and neonatal outcomes. Furthermore, the findings will contribute to the global discourse on maternal health by offering evidence-based recommendations applicable to similar resource-constrained settings.

1.2 Statement of the Problem

Maternal mortality remains a pressing global public health challenge, with approximately 295,000 women dying annually due to pregnancy-related complications, the majority of which are preventable [3]. Sub-Saharan Africa accounts for two-thirds of these deaths, with a maternal mortality ratio (MMR) of 534 per 100,000 live births, significantly higher than the global average [8]. In Cameroon, the maternal mortality rate is estimated

at 529 per 100,000 live births, underlining the dire state of maternal health services in the country [5] A key contributor to high maternal mortality is the inadequate management of pregnancy-related risk factors, including hypertensive disorders, hemorrhage, infections, and obstructed labor [9]. While antenatal care provides a critical opportunity to identify and address

these risks, resource-limited settings like Cameroon face persistent barriers, including shortages of skilled healthcare professionals, limited access to essential equipment, and inconsistent adherence to evidence-based practices [10].

In most hospitals in Bamenda, the risk seems to be compounded by high patient-to-provider ratios, limited medical supplies, and the socioeconomic constraints of the local population. Pregnant women in this region often face delayed access to care, increasing the likelihood of complications. Despite the critical role of nurses in delivering antenatal care, there is limited evidence on the effectiveness of their practices in managing pregnancy-related risk factors at this facility. This study addresses the critical need to evaluate and improve nursing practices specifically in the management of pregnancy risk factors within hospitals like Nkwen District Hospital. Recognizing existing gaps in current practices and exploring barriers to the effective identification, assessment, and mitigation of these risk factors, this research aims to provide evidence-based recommendations to enhance maternal health outcomes, particularly in resource-limited settings. The findings will not only contribute to reducing maternal morbidity and mortality locally by optimizing the management of conditions like pre-eclampsia, gestational diabetes, and infections, but will also inform broader healthcare policies and interventions in similar contexts. Ultimately, this research seeks to empower nurses with the knowledge and tools necessary to proactively manage pregnancy risk factors, leading to healthier pregnancies and improved maternal and fetal well-being.
