Key findings underscore the pivotal role of ITNs in mitigating malaria transmission during pregnancy, thereby significantly reducing maternal and fetal health risks. Through a comprehensive analysis of socio-economic factors, educational initiatives, and community awareness programs, this study sheds light on the determinants influencing ITN accessibility and utilization rates among pregnant women.

Furthermore, this document elucidates the multifaceted challenges encountered in the implementation of ITN interventions, including logistical constraints and behavioral barriers. Chapter One: Introduction

1.1 Background of the Study

Malaria remains a significant public health challenge globally, particularly in sub-Saharan Africa, where pregnant women and children under five years old are disproportionately affected (WHO, 2020). In endemic regions like the Buea area of Cameroon, malaria poses a substantial burden on maternal and child health, with adverse outcomes such as maternal anemia, low birth weight, and infant mortality (Desai et al., 2007). In response to this health crisis, the World Health Organization (WHO) has advocated for comprehensive malaria control strategies, including the promotion of insecticide-treated bed nets (ITNs) as a cornerstone intervention (WHO, 2019).

ITNs have demonstrated effectiveness in reducing malaria transmission by providing a physical barrier against mosquito bites and by killing mosquitoes that come into contact with the treated netting (Lengeler, 2004). Pregnant women are particularly vulnerable to malaria due to immunological changes that occur during pregnancy, increasing their susceptibility to infection and complications such as severe malaria and maternal death (Desai et al., 2007). Consequently, the use of ITNs during pregnancy has been recognized as a crucial preventive measure to protect both the mother and the unborn child from malaria-related morbidity and mortality (WHO, 2018).

Despite the proven efficacy of ITNs, challenges persist in achieving universal coverage and consistent utilization among pregnant women in malaria-endemic areas. Socio-economic factors such as poverty, limited access to healthcare facilities, and lack of awareness about malaria prevention strategies contribute to disparities in ITN ownership and usage (Singh et al., 2018). Additionally, logistical barriers such as inadequate distribution channels and stock-outs of ITNs further hinder access to these life-saving interventions (O’Meara et al., 2010).

Moreover, behavioral factors including misconceptions about ITN efficacy, discomfort associated with sleeping under nets, and cultural beliefs may deter pregnant women from using ITNs consistently (Rassi et al., 2017). Addressing these multifaceted challenges requires a comprehensive understanding of the socio-cultural, economic, and healthcare system contexts within which ITN interventions are implemented.

In the Buea region of Cameroon, where malaria transmission is endemic, the utilization of ITNs among pregnant women remains suboptimal, despite concerted efforts by government and non-governmental organizations to promote their uptake. To address this gap in knowledge and inform evidence-based interventions, this study seeks to investigate the utilization of ITNs and its effects on the prevalence of malaria among pregnant women attending the Buea Regional Hospital. By examining the distribution dynamics, accessibility challenges, and socio-cultural determinants influencing ITN utilization, this research aims to provide insights for improving malaria control strategies and optimizing maternal and child health outcomes in the region.

1.2 problem statement

In the Buea region of Cameroon, despite concerted efforts to promote the use of insecticide-treated bed nets (ITNs) among pregnant women, malaria remains a significant public health concern, adversely affecting maternal and child health outcomes. Despite the known effectiveness of ITNs in preventing malaria transmission, there persists a gap in understanding the factors influencing their utilization among pregnant women attending the Buea Regional Hospital. This gap hinders the development of targeted interventions aimed at improving ITN coverage and adherence, thereby limiting the potential impact on reducing malaria prevalence and its associated adverse outcomes among pregnant women and their infants.

Furthermore, socio-economic disparities, logistical challenges in ITN distribution, and behavioral barriers may exacerbate the low uptake and inconsistent use of ITNs among pregnant women in the region. The limited access to healthcare facilities, coupled with inadequate awareness about malaria prevention strategies, further compounds the problem, contributing to the persistence of malaria-related morbidity and mortality among this vulnerable population.

Addressing these challenges requires a comprehensive understanding of the contextual factors influencing ITN utilization among pregnant women in the Buea region. By identifying the barriers and facilitators to ITN uptake, as well as exploring the socio-cultural and economic determinants shaping pregnant women’s decisions regarding ITN use, this study aims to inform targeted interventions that can enhance ITN coverage and adherence, ultimately reducing the burden of malaria among pregnant women and improving maternal and child health outcomes in the Buea area of Cameroon.

  3. What are the socio-economic, cultural, and logistical barriers to the consistent use of ITNs among pregnant women in the Buea region?
  4. How do knowledge, attitudes, and perceptions about malaria prevention and ITN utilization influence pregnant women’s decision-making processes?

Objectives

  1. To assess the current utilization rates of ITNs among pregnant women attending the Buea Regional Hospital.
  2. To determine the prevalence of malaria among pregnant women who consistently use ITNs compared to those who do not.
  3. To identify the socio-economic, cultural, and logistical factors influencing the consistent use of ITNs among pregnant women in the Buea region.
  4. To explore the knowledge, attitudes, and perceptions of pregnant women regarding malaria prevention and ITN utilization.
  5. To provide evidence-based recommendations for improving ITN coverage and adherence among pregnant women in the Buea region, with the aim of reducing malaria prevalence and improving maternal and child health outcomes.
